Name Change Stuff

If the Chapter is Tax-exempt under Federal Law: File IRS Form 990-EZ, "Short Form Return of Organization Exempt from Income Tax." Under Box B check "name change." You do not need to include the financial data requested on the remainder of the Form. Instead, attach the documents require by the directions on page 16 of the IRS "Instructions for Form 990 and 990-EZ." These forms and instructions can be downloaded from the IRS website, www.irs.gov. Click on "Businesses," then "Forms and Publications" (under Resources), then "Forms and Instructions."

The instructions for the 990-EZ state that if the tax-exempt organization has changed its name and is a corporation, it should attach to the Form 990-EZ a copy of the amendment to the articles of incorporation, "with proof of filing with the state for incorporation." Also, a copy of the amendment to the constitution, by-laws or other organizing document, "along with the signatures of at least two officers/members." For completeness, also attach a copy of your original "determination letter" from the IRS.
